AYURVEDIC MANAGEMENT OF CHRONIC PANCREATITIS :-
जीर्ण-अग्न्याशय-शोथ की आयुर्वैदिक चिकित्सा:-


Chronic pancreatitis (जीर्ण-अग्न्याशय-शोथ) is not a very common disease. There exist nearly 100-200 patients in 1 lakh population in India. However, it needs special attention because of several reasons -

• It affects primarily the men in their prime of age, between 20-40 years, though women and children get affected, too;
• It is a quite painful disease that (at times) requires powerful painkillers for pain relief;
• Many patients suffering from this disease may remain undiagnosed;
• Two thirds of the patients are alcoholics; and
• In addition to pain, it results in several complications that haunt the patients for the rest of their life.

Causes (निदान):-
Following are the most common causes of chronic pancreatitis -
• 60% cases result from Alcohol (मद्यपान);
• 20% cases result from unknown causes; and
• 20% cases result from many causes - auto-immunity, gall stones, infections, trauma, metabolic disorders, surgery, high triglycerides, hereditary disorders.

Symptoms (लक्षण):-
Chronic pancreatitis usually results in the following symptoms and complications -
 
1. Abdominal  pain (उदरशूल), that at times could be too severe. It is most commonly felt in the epigastrium, that quite often radiates to the back. It is usually described as boring, deep, and penetrating, and is often associated with nausea and vomiting. It aggavates after meals and during night.  
2. Steatorrhea (diarrhea - अतिसार) with weight loss (कृशता); and/or
3. Diabetes (मधुमेह)

Management (चिकित्सा):-
The management of chronic pancreatitis could be customized as under -

1. Pitta-dosha-vipreet (पित्त-दोष-विपरीत):- Drugs:
Being a pitta-sthana disease and inflammation as the dominant pathology, it is prudent to take recourse to pitta-shamak drugs. 

Commonly used drugs for this purpose, include -

i. Bhringraj, 
Madhuyasti, 
Shati, 
Mukta-shukti
ii. Guduchi, 
Amalaki, 
Shatavari
iii. Abhrak, 
Rajat, 
Mukta, 
Praval, 
Suvarna

2. Anti-inflammatory (शोथहर) / Analgesic (वेदनाहर) Drugs:
Inflammation being the predominant pathology, it needs to be managed with anti-inflammatory drugs. 
The commonly used anti-inflammatory drugs are -

i. Shallaki, 
Erandmul, 
Jatiphal , 
Guggulu, 
Dashmul, 
Rasna, 
Haridra etc to manage the inflammation;

ii. Jatiphal, 
Godanti, 
Vatsanabh, 
Tagar, 
Pippali-mula, 
Parsika-yavani to relieve pain;

iii. Tagar, 
Brahmi , 
Jyotishmati, 
Akarakara, 
Vacha, 
Gandira  
could be combined with the drugs in the first two groups, when there is associated anxiety and/or depression.
    
3. Immuno-modulator (आमविषहर) Drugs:
In case the cause is the underlying autoimmune abnormality, following immuno-modulator drugs could be used -

i. Ashwagandha, 
Guduchi, 
Tulasi, 
Pippali, 
Yashad ;
ii. Amalaki, 
Haritaki;
iii. Punarnava, 
Katuki;
iv. Suvarnaprashan drops

4.  Antioxidant (रसायन) Drugs:-
Antioxidant drugs when used, 
may benefit in painn relief, prevent further progression of the disease, and healing. Commonly used antioxidant  drugs include -

i. Antioxidants (रसायन औषधियाँ) -  
Shilajatu, 
Amalaki, 
Mukta-shukti, 
Svarna-makshik, 
Abhrak, Yashad 
ii. Minerals & vitamins - 
Mukta-shukti, 
Amalaki, 
Abhrak, 
Yashad 

5. Liver tonics (यकृत्-रसायन):
When alcohol is the underlying cause of chronic pancreatitis, liver tonics and liver protective drugs chould be used. Commonly used liver tonics and hepato-protective drugs include -

i. Bhumiamalaki ;
ii. Kakmachi, Sharpunkha, Bhumi-amalaki 
iii. Punarnava, Kasni, Katuki

6. Anti-infective (संक्रमणहर) drugs:-
When infection is the underlying cause of chronic pancreatitis, anti-infective drugs could be used. Commonly used ayurvedic anti-bacterial drugs include-
i. Chirayata, 
Ativisha, 
Gandhak,
Yashad ;
ii. Guggulu, 
Haridra;
iii. Manjishtha, 
Sariva, 
Nimba; 
iv. Shilajatu, 
Guduchi 

7. Surgery (शल्य-चिकित्सा):-
When the underlying cause is gall stones, tumors, or some other obstructive pathology, surgery should be advised.
